&lt;p&gt;Anal sacs can be an issue, g;ands are long,deep and tubular so previous clinic did not empty them fully-that is now well controlled and was at the time of this new pattern.&lt;/p&gt;
&lt;p&gt;The licking was manic and excessive and nothing like normal itch-lick pattern.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;Stress/Behaviour could indeed have been a trigger-one owner fell over and broke hip, the other suddenly died.&lt;/p&gt;
&lt;p&gt;gabapentin is being trialled but renal levels were up soneed to&amp;nbsp; using lower doses.&lt;/p&gt;
&lt;p&gt;The dog needs also to lose weight as has hooded vulva and intermammary folds and responded well currently&amp;nbsp; to good clean and scrub and application of Sudocrem nappy rash cream and Kefvet antibiotics.&lt;/p&gt;
&lt;p&gt;Dog had been to specialist derm for the whole das blinkin lights investigation./slit/id etc.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;Agree re referred pain in anal sac licking so L/S disease always a concern.&lt;/p&gt;
&lt;p&gt;The best response&amp;nbsp; for the 6 yr long atopy-went on low allergen diet for the &amp;#39;Atopy&amp;quot;-this year on the diet,&amp;nbsp; needs hardly any skin issues to address-other than this new sign&lt;/p&gt;
&lt;p&gt;The cortex was&amp;nbsp; abnormal but follow up scan shows normal cortex now.&lt;/p&gt;

&lt;p&gt;I agree that a repeat ultrasound with a fuller bladder would be appropriate to assess that thickening further- it does appear slightly thicker cranially, but it&amp;#39;s impossible to say if its pathological or not.
