12 year old female neutered Persian cat. Presented with increased respiratory effort over the last 2 weeks, some wheezing present, but heart no obvious murmur. Started on frusol while getting in for xrays.

I'm not a great ultrasonographer, but I've been surprised at how easy it is to get something from a basic scan of the heart- just the other week I diagnosed an unexpected pericardial effusion, which I managed to drain! Cats can have HCM without a murmur, I believe. I would be looking at Left atrial size, and I have managed to see thrombi and "swirling" on basic ultrasound before. Apparently troponin 1 is quite a useful marker in cats as well.

RE: Pleural effusion Xray
Have you put the ultrasound scanner on the heart?
Agree.

Trachea looks a bit elevated and pulmonary vessels look enlarged. Worth popping the u/s on the heart before doing CT scan IMO. That could give you the answer.
If you're not confident with ultrasound running proBNP might be helpful to determine if it is cardiac or not, and you can ask the lab to run it on the pleural effusion.

RE: Pleural effusion Xray
