I often find the lower lid tends to sneak under the upper at the lateral canthus and is often too long so I often combine Hotz - Celsus with a lateral wedge resection of the lower lid. Difficult to get them all just right and if I have do fine tuning I usually do it at cost price having explained to the owner that if they look great at surgery it doesn&amp;#39;t guarantee 100% resolution a few weeks later. It really is an art rather than science and with some post-op swelling if they don&amp;#39;t look like a mild ectropion immediately post-op I am concerned that I have been too conservative.&lt;/p&gt;

&lt;p&gt;I would think about some temporary tacking sutures to relieve the entropion but I also suspect that it still needs a bit more skin removing as blepharospasm on its own shouldn&amp;#39;t induce entropion it just aggravates pre-existing conformational issues. Even when they resolve with topical anaesthesia it doesn&amp;#39;t mean that further surgery is not required.&lt;/p&gt;
