I suspect entropion is the primary condition. I often find the lower lid tends to sneak under the upper at the lateral canthus and is often too long so I often combine Hotz - Celsus with a lateral wedge resection of the lower lid. Difficult to get them all just right and if I have do fine tuning I usually do it at cost price having explained to the owner that if they look great at surgery it doesn't guarantee 100% resolution a few weeks later. It really is an art rather than science and with some post-op swelling if they don't look like a mild ectropion immediately post-op I am concerned that I have been too conservative.

I don't think a 3rd eyelid flap will solve this problem.

with Rotties it often helps to get the plate that looks like the top of action mans skateboard, on your eye kit and push it into the lateral cantus as there is often an overtight lateral canthal ligament that carries on pulling the lower lid over laterally. If you haven't already cut this it may be worth sniping it .

Hi Glen

I would think about some temporary tacking sutures to relieve the entropion but I also suspect that it still needs a bit more skin removing as blepharospasm on its own shouldn't induce entropion it just aggravates pre-existing conformational issues. Even when they resolve with topical anaesthesia it doesn't mean that further surgery is not required.
