I guess if clinical signs abate post-op you’ll have your diagnosis (not Addisons!) if not retest once been off preds for a while. Good luck!

RE: Addisons

Was being treated as atypical with Prednisolone, some signs improved but then vomiting significantly worsened, xrays demonstrated large piece of bone in stomach (chicken thigh) which was removed via gastrotomy, question is was bone there before or just eaten the night before as polyphagic on steroids and owner did see dog get hold of something and took bone out of mouth! If the dog is ‘stressed’ - which I guess it will be if you‘re suspecting Addisons, then it should have a basal cortisol higher than that.
In the past I have just gone for basal cortisol to try and keep costs down for owners - no point as often I still had to do an ACTH stim test afterwards. I would go straight for this once some Synacthen or Tetracosactide has been sourced.

I agree - often they can send you some direct through your wholesaler. If not, they will help you find some.

RE: Addisons

Try speaking to Dechra, they are generally very helpful.

RE: Addisons

Have you done a basal cortisol? If greater than 55nmol/l then unlikely to be Addison’s - although you still really need to do an ACTH stim to confirm if less than 55nmol/l. Remember too that approx 10% of Addisonian dogs will be atypical ie no significant change in electrolytes. Definitely always worth ruling out early on - and definitely before you start steroids!
