The AF&amp;nbsp; was not confirmed by ECG but they certainly sounded like it on auscultation and had pulse deficit, plus the above signs. In any event they had severe cardiac disease so I became a big fan of the stuff. The ESVC have a list of criteria for calling DCM which may be worth looking up. One of the most sensitive measures I find is the sphericity index of the LV.&lt;/p&gt;
&lt;p&gt;What was the LA/Ao ratio? Have you done an ECG?&amp;nbsp;&lt;/p&gt;
&lt;p&gt;Pimobendan trial would be sensible given your signs. I wouldn&amp;#39;t chase the pulmonary hypertension tbh. &lt;p&gt;Is pimobendan not in theory of potential benefit to pulmonary hypertension?&lt;/p&gt;
