Seen this dog for post op check: healing amazingly well and is actually able to breathe through it's left nostril! Owner is delighted at the improvement in the dog's general demeanour, not to mention halitosis...

Would love to re-xray in a couple months, but not going to be possible.

Still, good outcome so far.

Given that the owner refuses any investigation....

It's between infection and neoplasia to my mind, and the response to the surgery will be informative.

I think I would have taken biopsies (keep them in the cupboard if owner doesn't want to send straight away in case the whole thing falls apart). There is obvious lots of odd-looking tooth resorption, but there is also massive bone loss around 202/203 with what looks on the radiograph like soft tissue swelling, and there is a fracture to the incisive bone lateral to the left palatine fissure. What did it look like grossly? Has blood calcium been checked?

U/C and SDMA wnr. Bone looks osteoporotic to me, hence the query about something more metabolic or genetic. I’ve seen some smelly mouths in my time but never anything as profound as this.

PS: both lower arcades fairly normal...

Well, it would be worth surveying a few other bones and doing a blood profile. Kidney function OK?

Looking at the resorptions on those roots, it could just be horrid advanced chronic infection. But you need to know, surely.
