Is only needed if a requirement of the carrier, usually in cases where you are handing over the care of the animal to a 3rd party e.g. airline cargo, pet shipping company in Europe. Who asked you for this?

From ET159 (APHA guidance on completion of passport) "Section X: Clinical Examination Whilst the entry for clinical examination is not required for the movement of dogs and cats to other EU countries, some transport companies may still require owners to produce evidence of an animal’s fitness to travel. OVs may, therefore, complete section X of the passport if they wish to do so."
asked to administer praziquantel
Only needed before entering or re-entering the UK (or certain other countries that have echinococcus controls) but the window is 24-120hrs before entry to the UK. So if I am going to France today and coming back in 2 days' time with my dog, I can do the tapeworm treatment in this country before I go and comply because this is in that window. Obviously the praziquantel will be out of the dog's system early into my trip in France and the dog may still be at risk of picking up echinococcus, so in such cases we would recommend repeating treatment (albeit the client can do at home) once they're back in the UK.
