&lt;p&gt;It could be pancreatic disease but TLI and PLI elevations can also be seen with inflammatory (or other) gut diseases so it may not be the primary issue.&lt;/p&gt;
&lt;p&gt;How old is the dog? Any vomiting? Abdo pain? Weight loss?&lt;/p&gt;
&lt;p&gt;Ideally an abdominal ultrasound might be useful here and, depending on findings, an exclusion diet trial might be a good starting point. I also supplement B12 at that level (I supplement anything less than 400 ng/L).&lt;/p&gt;
