I haven't taken up the new procedure lightly and yes, we have some of the suggested ideas to help our learning curve either in place or thought about.

We also want to initially start with CWO, rather than radial and gain more experience here before progressing.

My question was more about how to share the basics of this (and others if you wish) of cruciate surgery with the rest of the staff without having to write a paper or make a webinar myself. I'm sure there should be something out there.

I feel it is sometimes difficult to explain these procedures to clients and I realise many nurses (and vets!) do not grasp basic bio-mechanics of osteotomies to treat cruciate failure. Being this TTA, rapid TTA, TTO, TPLOs, MMP, etc.

If anyone knows about a webinar or, might be even, a short youtube video, it would be interesting to point them in that direction.

Thanks.

RE: TPLO

I perform TTO surgery. I have a fair bit of general orthopaedic experience and was taught the technique by a specialist with 40 years orthopaedic experience who judged that I was OK to fly solo. I was happy and relaxed after 20 or so and could phone my mentor with any queries. You need the relevant background experience to adapt to complications which you will encounter and the success depends on meticulous attention to detail. If I were to use a different technique I would still assist a specialist with experience for one or two operations before embarking on my own. The devil is in the detail !

hi David. I'd be interested in seeing the study. It is often said that the learning curve for TTA is less than TPLO - 23 cases seems similar to TTA. If you had a link, it'd be greatly appreciated.
Mea culpa, it was the TTA study I was thinking of. Malcolm Ness said the learning curve was similar for MMP (around 20 mark if I remember rightly).

Yes, I would imagine the TPLO would be higher as I understand it to be a more complex procedure.

RE: TPLO
TTA learning curve for one TTA technique and one surgeon published
I would recommend that you seek out an experienced TPLO vet willing to allow you to scrub in and ask if one of the nurses could come along as well. TPLO is a formidable procedure and while it produces excellent results, it is associated with pretty major complications and like any other surgical technique, these are more likely early in the learning curve. Seeing the live cases gives you a chance to notice how theatre is arranged, what will be needed in terms of patient positioning, instrumentation, assistance, nursing care, etc etc so that the operation itself becomes easier. Cadaver practice would be wise prior to engaging live cases and if you are lucky enough to have an experienced colleague willing to guide you through the first few, so much the better. There are some videos of the technique on youtube, but these are necessarily shortened. Hope that is of some use, sorry if you've considered all of this already.
