I suspect a lot is systemic absorption rather than local effects?

RE: Depo Medrone

Glad to hear that the gruesome slash down the medial surface seems to have died a sensible death.

I won't move on to cosmetic treatment of the total pinna haematoma.

RE: Depo Medrone
You must though WAIT 2 weeks, as they will swell again before dropping
A high proportion will not recur if you wait 2 weeks anyway (that's if you can drain them because they have an organised clot) as adhesions are already forming. In theory corticosteroids should reduce this healing response so if they do improve after Depo. it supports the auto-immune cause. That's not to say I never use Depo but logically I would do it at an earlier stage with the intention of it reducing inflammation an ongoing bleeding. I would wait 2 weeks if I was going to treat them surgically though.

The figures start to be a bit anecdotal but I'd say 50% of haematomas drained in the first few days and injected with Depo. won't recur whereas the majority which are just drained will.

Incidentally after my skiing injuries, the left shoulder which had an injection of Depo into the tendon of the infraspinatous muscle has improved much better than the right which as left untreated.

RE: Depo Medrone

My experience is that aural haematomas take (up to) 2 weeks to stop filling regardless.

RE: Depo Medrone

Thanks Neil

do you know the proposed mode of action?

ive heard irritation causing adhesions and anti inflammatory so a bit polar really.

RE: Depo Medrone

I did a case on this at the RVC 28 years ago. If I recall , 100 dogs were anaesthetised by the Japanese and their ears rubbed trying to induce one, they failed. As a result the paper suggested an autoimmune response.

Since then I have drained and injected hundreds of these and instilled 0.5ml DMV. As far as I'm aware only 2 have come back up again. You must though WAIT 2 weeks, as they will swell again before dropping and I write this in capitals on clinical notes as one locum follow up showed one was operated on a week later as the vet failed to read the notes or thought they knew better
