&lt;p&gt;I would say increase dose and test again 3-4 weeks in your case.&lt;/p&gt;

I had heard (but can&amp;#39;t remember where from) that bioavailability might actually be better when in food but not been trialled - certainly as long as consistent shouldn&amp;#39;t be making a big difference.&amp;nbsp; Hopefully a bigger dose will sort the cat out!&amp;nbsp;&amp;nbsp;&lt;/p&gt;

&lt;p&gt;as I said, I do think the cat is receiving the medication.&lt;/p&gt;
&lt;p&gt;I don&amp;#39;t like thyroidectomies for all the reasons already mentioned above, its my last resort option.&lt;/p&gt;
&lt;p&gt;I-131 is on the radar for the owner but she doesn&amp;#39;t want to do it because of the boarding issue.&lt;/p&gt;
&lt;p&gt;We have discussed y/d and as the hat is a solo homebody, it&amp;#39;s our next option&lt;/p&gt;
&lt;p&gt;But we are increasing the dose and rechecking in a few weeks.&lt;/p&gt;
&lt;p&gt;(the cat also has some pre-existing mild renal issues)&lt;/p&gt;

IME they are rarely anything other than a vaguely paler bit near the proximal pole and I have done dozens of them. It is then not straightforward to dissect out and/or preserve the blood supply especially if the thyroid is fragile and the capsule indistinct and there is no certainty that if you bury them in muscle they will survive. &lt;p&gt;I agree I like to feel the neck before deciding on options, however:&lt;/p&gt;
&lt;p&gt;If bilateral, you still have plentiful options to improve things:&lt;/p&gt;
&lt;p&gt;1) Just take out the bigger gland - complete extracapsular if you don&amp;#39;t plan on returning +/- cutting out the parathyroid and sticking it in neck muscle&lt;/p&gt;
&lt;p&gt;2) Staged removal 3 weeks or more apart&lt;/p&gt;

&lt;p&gt;[quote user=&amp;quot;Clive Ansell&amp;quot;]Or Iodine-131 therapy?, depending upon the case and the owners wishes and expectations of course.[/quote]Very much dependent, most owners don&amp;#39;t want to/can&amp;#39;t afford to pay or have some beef about the cat being away for 2-3 weeks and the last one I sent for I-131 therapy ended up hypothyroid, which made it hypotensive, which exacerbated existing renal disease and has to be very tightly controlled so hasn&amp;#39;t become any easier to manage. Plus, although its irrelevant to the current thread other than to show the OP&amp;#39;s case is relatively straightforward, it&amp;#39;s diabetic, obese, has autoimmune skin disease and the owner struggles to pill it - joy!&lt;/p&gt;

&lt;p&gt;I called the drug company to ask about this (because I don&amp;rsquo;t think giving liquids to cats outside of food is easy at all, half of it goes up the wall) and they said that anecdotally it seems to work fine but they don&amp;rsquo;t have data for it.&amp;nbsp;&lt;/p&gt;

&lt;p&gt;That said, I don&amp;#39;t like Carbimazole I&amp;#39;ve never found it as reliable or as adjustable as methimazole. &lt;p&gt;First place I would look is owner compliance; if they say cat is getting 95%, they probably mean 50% or 5%, or even the other cat or the second cousins twice removed dog is getting it.&lt;/p&gt;
&lt;p&gt;I have had a recent case where the owners insist that the cat gets it Vidalta every day, yet the last prescription for 30 days worth was collected in March! cat has lost weight, has HR&amp;gt;200 and T4 &amp;gt;150. Remainder of bloods and c/e all unremarkable. &amp;nbsp;&lt;/p&gt;&lt;div style="clear:both;"&gt;&lt;/div&gt;</description></item></channel></rss>
