If cost is an issue then BID vetoryl will also be a consideration! Just asking - can you take a pre-dose cortisol to see if there is a need for BID (as you would a diabetes curve) to see if there is a significant breakthrough on SID dosing or if current dose is maintaining 24hour levesl and have an additional issue also?

I wanted to keep the overall dose the same initially, and just split to twice daily.

Only last week and may this year, USG 1.002. other entries in the notes are limited too "urine dilute"

other relevant history could be a mammary mass removed > 2 years, confirmed by histology as a mixed benign mass. No sign of regrowth, no wt loss (overweight), and no sign of metastases on chest and abdominal rads.

Diagnosis was based on clinical signs, and a post ACTH cortisol >800. did respond well to treatment initially.

Full urinalysis last week was all wnl other than an SG of 1.002. Nothing found on C&S.

What is its post-ACTH cortisol? If you are increasing the Vetoryl dose but you can't source smaller tablets would it be any use to dose 10mg and 20mg on alternate days? With a USG that low I would agree that concurrent diabetes insipidus is a distinct possibility. Were it's clinical signs previously well controlled on the same dose of Vetoryl? And do you have records of what its USG has been in the past?
