&lt;p&gt;Their QOL is greatly increased with the switch to Torasemide and it is not prohibitively expensive. This is not something which may or may not work better like Pimobendan v Sildenafil, it is something which does work which is replacing something which has become ineffective.&lt;/p&gt;

&lt;p&gt;[/quote]I know you will never accept my experience but it has worked very well in the limited cases I&amp;#39;ve used it in. I didn&amp;#39;t pluck it out of the air, several highly qualified speakers at CPD events have recommended it.&lt;/p&gt;
&lt;p&gt;The adverse reactions list in the BSAVA formulary does make interesting reading but I&amp;#39;ve not seen any adverse electrolyte/biochemical effects, in fact renal function has improved and it seems very counter-intuitive to warn it can have an adverse effect on cardiac function when its primary use is for furosemide refractory CHF!&lt;/p&gt;
&lt;p&gt;It is a case of swings and roundabouts and as said the few cases I&amp;#39;ve used it on have all benefited: RRR lowered and cough due to pulmonary congestion disappeared when one dog was previously on 10mg/kg Frusemide + spironolactone.&lt;/p&gt;
&lt;p&gt;Unlike Sildenafil I&amp;#39;m not tempted to use it as a first line treatment and your suggestion that the patients are better off dead is facetious. Maybe for your charity cases but for paying clients who want to improve the quality of life of their pets it is genuinely useful. I have given it to a couple of dogs which have nearly emptied the pharma cupboard and its given them a new lease of life. &lt;p&gt;It actually comes from the human field but has the unfortunate side effect in humans of gyanecomastia in about 25% of patients which is undesirable. There is a similar drug levosimendan which is commonly used.&amp;nbsp;&lt;/p&gt;

I started using Vetmedin as soon as it was launched on a dog that was in near terminal CHF on ACEi, spironolatcone and diuretics I made an appointment for it to go to the RVC and by the time it got there it was 90% better. They sent it back saying they couldn&amp;#39;t do any more, at that point even they had not started using it. it lived for another 2 years.&lt;/div&gt;

&lt;p&gt;I&amp;#39;ve always been a bit cynical regarding pimobendan, primarily because I&amp;#39;ve never heard anything bad about it, it appears (in my limited experience) to be wholly benign when given for years to patients that didn&amp;#39;t clearly need it (a reverse sneezing dog comes to mind), even after years of administration it can apparently be abruptly stopped with no obvious detrimental rebound effects, I can&amp;#39;t &amp;quot;see&amp;quot; its effects clearly and obviously in patients I treat with it, it is a wonder-drug for all domestic species (but no drug in its class has performed well in large-scale human trials despite numerous attempts as I understand it?). Part of me is cynical and part of me thinks it should be in the drinking water.&lt;/p&gt;

I was the first vet other than those involved in the trial who knew the results of the EPIC trial as I as told 5 minutes after the results were officially released in 2015 by Mark Patterson on SPVS Snowscene. Now that is what you call up to the minute CPD! &lt;p&gt;I am a huge fan of preemptive treatment in cardiac disease and the EPIC trial has only served to confirm my feelings on that. However it applies more to dogs with cardiomyopathy not mitral valve disease and if the dog is not showing any signs of cardiac failure &amp;nbsp;due to that then I cannot see it would be of benefit until it needs to be part of management of CHF. An NT-proBNP may give you a bit more of an idea about that if you&amp;#39;re not happy with your echocardiography.&lt;/p&gt;

&lt;p&gt;Anyway. It&amp;#39;s not endocarditis because the dog isn&amp;#39;t ill. Concerning that you can spot a lesion on mitral valve but couldn&amp;#39;t measure ratio? It could have been an orientation issue.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;Although not unheard of in boxers, mvdd would not be top of the list.
