I was chatting to the Zoetis rep this morning about Cytopoint. I asked about its use in my diabetic cat patient with allergic dermatitis and she thought it would be a useful drug under these circumstances but its not licenced for cats, not just because they haven't got the data yet but apparently lokivetmab is produced in mouse tissue culture then 'specieised'. Horrible word but it has to go undergo some process to 'caninise' it and it would just be rejected by the cat's immune system. However they are working on it so something for the future maybe?

Well intermittent steroid dosage as necessary, rather than a continuous regime, for a start, and earnest attempts to eliminate the allergen.

Certainly can do - I think it would be a balance of which would do so more, the atopica or steroids (if skin problems required treatment with one or the other. Bring on more useful treatments for allergic disease in cats, I say!

RE: Pemphigus in a diabetic

I was chatting to the Zoetis rep this morning about Cytopoint. The autoimmune skin disease affects its feet mainly and facial skin, the owner is incapable of giving any medication even topically. Against my better judgement (I've waxed lyrical on here before about the link between steroids and DM in cats), in desperation I gave it a course Depomedrone injections and this became a self-fulfilling prophecy and precipitated its DM. Its been weaned off the Depo for nearly a year now and its skin is tolerable but the DM hasn't resolved. The owner has a bottle of Cortavance if necessary but I doubt she could use that effectively either. Not sure what I would be doing if the skin problem comes back with a vengeance but I guess we'll have to reach for the cyclosporin - at least that comes in a liquid form.

These cases are always a challenge, which I enjoy as it makes life more interesting, but in the end with animals with multiple issues you've got prioritorise the treatment the condition which is causing the most problems even if that means a compromise.
