I would leave this, treat purely conservatively. It sounds like smegma, albeit being produced in larger amounts, but it is harmless, non-pathological and doesn't indicate infection, so I'm not surprised it hasn't changed with the medical treatment so far. I guess if the owners are really concerned you could suggest gentle saline irrigation but I certainly wouldn't be using topical antibiotics or antiseptics. The aim here is to let the normal flora re-establish - I bet once the puppy matures it settles down to a reasonable, normal level.

That's a very difficult distinction to make, I'd be suspicious the owners are putting two and two together (the discharge and the inappropriate urination) and making five. It's only 10 weeks old, of course it's going to pee indoors occasionally.
