He had 10mg/kg tid iv.

liver enzymes similar today alt 600+ alkp 1900, ggt 16, pcr 51%, Toral bilirubin 57,

i relented last night as he looked so miserable and gave a single dose of dexamethasone at the equivalent of 1mg/kg .Dog brighter and afebrile today still with stiff neck. Still waiting for Leishmania pcr on joint fluid

What was the dose of paracetamol? I've not seen toxicity at therapeutic doses before. Along with everyone else's suggestions I'd look into the bilirubinemia a little more. I take it from the scan the gall bladder was OK? Have you repeated the urinalysis and full blood count / red cell parameters (+ smear) to look for evidence of haemolysis?

Yes, both blood smears and serology and pcr on blood . Awaiting leishmania pcr on joint fluid

Did you look at a blood smear for Babesia and Hepatozoon, and a lymphnode smear for Leishmania and Ehrlichia?

To be fair they live in France not in the UK.

Wondering if paracetamol could have caused the icterus and high liver enzymes esp the ALKP. Seems to have ended up on synulox, marbocyl, metronidazole and paracetamol?

It's not just the bone-headedness but then the expectation of discounted or free treatment because of their virtue-signalling act (and they've been fleeced by an importer).

Most of these Romanian ones are quite happy street pack dogs plucked from the streets and then forced into a Western household. There was an article recently in the Times that said they were more likely to bite their owners or have behavioural problems. Go figure.

Hopefully Brexit will rectify the situation...

Leishmania until proven otherwise. Pain in the joints is common, and pyrexia present in around a third of cases. Also can affect most abdominal organs, so diarrhoea +/- nausea/vom also normally present. Can cause quite significant muscle inflammation and neuralgia too.

Hold off the preds for now, but some people do use them at anti-inflammatory doses to improve symptoms.

Prognosis if leishmania is poor long-term, and sounds like kidney involvement, if so then hopeless. Why people rescue these dogs belies belief.
