Thank you both, if I can catch it discharging I'll check for Demodex but otherwise I think the owner will be happy to hear we can monitor it!

Meibomian gland cyst/infection. Probably self limiting.

You can get pustules with demodex so if one of them bursts again it might be worth looking at the contents under a microscope, or if you're feeling very brave you could take a scraping from it.
