&lt;p&gt;I also agree with the suggested cause, and it seems to me control will be more likely than cure. I would completely agree that this doesn&amp;#39;t seem consistent with the appearance of a histiocytoma. Generally speaking they exfoliate very well and give you a large population of round cells with a high nuclear to cytoplasmic ratio and commonly a concurrent population of lymphocytes especially as they approach resolution.&lt;/p&gt;
&lt;p&gt;I also agree that the majority of cells are pmns with some other inflammatory cells thrown in as well. I wonder if what you are seeing here are multinucleated giant cells. They can be seen for a number of reasons including ironically histiocytic disease but usually are a feature of chronic inflammation and in my experience &amp;nbsp;possibly secondary to some kind of fb reaction. Now from the history a fb seems unlikely but perhaps a hair follicle has been exposed etc and is acting as such. Either way a. I think this cytology points more towards an inflammatory/infectious cause and b. I&amp;#39;m not really sure I&amp;#39;ve helped you get much closer to an answer for your patient!
