&lt;p&gt;Pulmonary lymphatic positioning - they are near the visceral pleura. CHF causes lymphatic congestion, so excess fluid gets dumped into pleural space.&lt;/p&gt;
&lt;p&gt;If you have ultrasound capabilities, you can pick up pulmonary edema and pleural effusion in &amp;lt;60 seconds. Check out the publications on TFAST. Very helpful, fast, and much safer than restraining for radiographs.&lt;/p&gt;

&lt;p&gt;If you&amp;#39;re draining pleural fluid anyway, and test that, I think it&amp;#39;s complementary rather than better - a low NT-proBNP result would make me go back and look again if I&amp;#39;d thought was heart failure or would reassure me I probably wasn&amp;#39;t missing something if I&amp;#39;d thought wasn&amp;#39;t heart failure.&lt;/p&gt;
&lt;p&gt;Given that, in my limited experience, neoplastic and cardiac causes of pleural effusion both have a fairly short-term outlook and risk of sudden fatal dyspnoeic suffering within a couple of months, I often euthanase these patients anyway irrespective of precise diagnosis. Equally, I&amp;#39;m probably going to treat with furosemide irrespective if not euthanasing, especially in older patient.&lt;/p&gt;
