I had an 7.5 week old pug cross presented to start vaccinations at the start of Dec. However his upper right lip had swollen up the day or two before coming in to us. On exam the lip was a bit crusty

RE: Young puppy with continually swollen lip....

What did happen?

He first presented on 28th Nov, that's when we tried synulox. He came back 10 days later on 8th December. We discussed further investigation and I said I would try and discuss the case with colleagues (as I had no clue at that point...!). I rang the owner after deciding FNA might be useful, and we ended up doing this on 22nd Dec. Based on the results, I started him on topical treatment (isaderm), I've shied away from systemic steroid injections, because rightly or wrongly we started his vaccination course when he came back in on 8th Dec (given that he was so bright and systemically well). So at this point in time we are now a month on from initial presentation and I'm due to see him back in a couple of days for his final L4 injection and re-ex after a week of isaderm.

For those of you voting for steroids, does this change things at all? What steroid injection would you give and what dose?

Any other suggestions still welcomed.

RE: Young puppy with continually swollen lip....
I'd be surprised if Cortavance would penetrate enough to be effective.
Contraindications

Do not use on cutaneous ulcers.

CAUTIONS: Total body surface treated with CORTAVANCE® Topical Spray should not exceed a surface corresponding to, for example, two flanks from the spine to mammary chains including the shoulders and thighs (1/3 of dog’s body).

This from the Canadian manufacturers website which seems as if it could be used on this pup as it sounds as if it is effective on intact skin?

RE: Young puppy with continually swollen lip....
+1, [surprised!] puppy pyoderma is always [?] multilesional with some purulent discharge and painful.

Probably an insect bite or sting, [even without FNA....!] It would be odds on to be that, and odds on to resolve with time, but quicker and less discomfit with short acting corticoS and

"say something, do something, give something...." so the owner's satisfied

Ever been bitten by a March fly......?

I wouldn't rush into a FNA in a little pup with that sort of lesion, but times change.
