I like chlorambucil in these cases as you often don't need such a high pred dose in an already overweight cat.

Thanks Thomas. I'll do 2 mg per kg bid so 10 mg twice daily.

My first thought would be to increase the pred dose up to 2mg/kg. Most of the other drugs you could add in, and I'd probably choose cyclosporin, are going to take time to have any benefit so you need to get it under control with the pred even if you're starting something else as well.
