The more I think about this, the more I think you're right. Looking at the pictures you sent on the link matches what I saw. The ulceration on the foot was huge (and new)

The owner just didn't want it investigated, but if anyone sees a skin case like this then maybe?

No testing for Leishmaniasis; pity, sounded the most likely and the pictures matched.

Note E for foot lesion and C for a weird case, with a photo of vesicles on skin [can't remember to link it with the post and thread] hit link below

I put him to sleep today

I can honestly say I've never seen anything quite like it.

Areas of granulation tissue on the inner thighs, one 5 cm wide, like a golf ball, red and quite angry.

Ulcerated areas on the face, the testicle area was reddened, swollen and ulcerated.

The weirdest was a huge ulcer on the main stopper pad on the right taking over maybe 1/3 of it. This would rule out a lymphoma?

I offered referral but the owner asked if realistically it could be controlled, I feel it was a brave decision and I agreed.

Will do next week, when back at the practice.

Leishmania is a thought as is the allergic (it has gone on for many years) I'm not sure if the owner was posted to Southern Europe but will ask.

I have asked the client and he is happy for me to post this here, so will try to update it.

Nothing I've ever, ever, seen and not one for Preds without histo "workup", for once.

Some sort of reaction to insect bite/parasite but inguinal area doesn't fit??

How does a lesion start. See if you can biopsy one in the very early stages.

Histology from edges of fresh lesions?? [The pictures look "mature lesions and "secondary changes"].

I suppose bloods [for once] might give you high eosinophils etc.?

? Shave an area to look for "fresh lesion"?

I suppose steroids will be the only option at the end anyway, and a trial can't hurt. If mine I'd use depomed. Preds won't give you a definitive rapid improvement IMHO.

One for the weird, wonderful, and rare!

Don't do the usual and go silent on us!!!

PS just couldn't be reactions to bed bugs could it; right hidden site and possible, I suppose???

Autoimmune/allergic.

Needs preds. It'll resolve in 2 weeks or so.

Yes drug reaction was a thought, but he has been on no treatment.

He has travelled about a bit as belongs to an army family.

Lymphoma is up there as well, but very long term. It's the recent lesions on the anus that is a worry

Calcinosis cutis usually is identified in histology isn't it? It looks like some pictures I've seen of drug reactions. Following this thread as I'd be interested to find out what this is.

Biopsy didn't show any tumour cells then? Epitheliotropic lymphoma would be up there for me (it's a boxer after all...)

On any other meds? Some sort of crazy drug eruption? Has it always been this bad? Any biochem/haem abnormalities or systemic illness? Any travel history? Hopefullt a derm enthusiast will be along soon...
