My understanding is that I can't determine if this may be benign or malignant from cytology. It is mildly ulcerated. I have discussed castration + possible surgical

RE: Anal adenoma medical treatment options?

I typically castrate these dogs (mainly based on cost) and advise to come back in 2 to 3 months by which time usually perianal mass has subsided largely - if still needing surgical removal, it will be a lesser job i figure.
Surely two procedures will coast more to say nothing of 2GAs, double the follow-ups etc.??

Tardak always shrunk the tumour IME and confirmed the diagnosis too, so a win for me and the dog and less dosh for the owner.
Castration has historically cost the owner £15 (irrespective of size) under the dogs trust neutering scheme (though not all are now eligible since changes earlier this year). In the majority of cases this leads to complete resolution without any further treatment.

A bottle of tardak costs significantly more than £15 last time I checked. Obviously if you get through it in a month then can split cost between lots of patients, but personally I haven't been in a clinic in last decade that used a full bottle in 28days reliably. And it only lasts about a month after which you still need castration or repeating it regularly etc... so just castrating in first instance is definitely cheapest option here - I don't stock Tardak, if I did then I would feel the need to "use it up" and may feel differently.

I'm not opposed to excising peri-anal masses at same time as castrating, but I tend to do all my castrations prescrotal/scrotal and it adds faff time to prep two areas, turn dog round etc + increased risk of faeces related wound infections, so if it's not actually necessary in majority of cases, I personally find it less effort to castrate them all first and only bother with excising masses where required.

If it's a female or neutered male I excise the mass first-off.

RE: Anal adenoma medical treatment options?

Castration and stilboestrol would be pretty effective. or Oestradiol injection if stilboestrol isn't available? So hormonal control is the key I believe.

RE: Anal adenoma medical treatment options?
I typically castrate these dogs (mainly based on cost) and advise to come back in 2 to 3 months by which time usually perianal mass has subsided largely - if still needing surgical removal, it will be a lesser job i figure.
I used to give them Tardak just to prove to the client that castration would not affect the dog's self-worth or esteem, nor reflect on the male owner and, totally irrelevantly,by the way, to make sure castration would work, prior to an easier removal.

Or sometimes, with a sensible owner, castrate and excise concurrently, particularly if the mass was leaking all over the persian rug......

RE: Anal adenoma medical treatment options?

Hi,

Thanks for starting this topic! I have a similar case in a castrated 10 y old Cocker Spaniel. I was looking for articles, but most of them are about intact males. Millan et al. (2013) mention that oestrogen therapy in neutered females can be an option, so I was wondering does it makes sense in castrated males as well (it seems like the androgen-receptor mechanism is the same, just responding to hormones from the adrenal glands cortex)? Has anybody tried Deslorelin/Tardak/Ypozane in a castrated male?

Many thanks!

I would use suprelorin (the 6mth implant) if not wanting to surgically castrate, though if you stock tardak then an injection of that and follow-up in a month would be fine also i think.

I don't know if there would be any additional benefit to ypazone in addition to suprelorin? Maybe others can advise on this one.
