I would worry about seeding cells along your injection path doing this but have never done it so cant say how effective it was/is.

I do think we forget about the published efficacy of preds in these mast cell cases and believe more early case would do better if given g/c from the get go-go many now fluffying around trying to decide which new expensive antimast drug to use, often which owner can't afford when dog could have been on preds and resolving in all of that time.

Re lick granulomas-was still going in the 80s/early 90s to inject the lesion with g/c- I wasn't overly convinced re efficacy so didn't use the technique for the lick granulomas.

RE: Perianal subcutaneous mast cell tumour
corticosteroid
It's a good and important question. My experience is that Masivet and Palladia are most effective in the very aggressive-looking mast cell tumours. I don't choose to use them in the lower-grade tumours. Sometimes, I understand there is a drive to look for a medical therapy because, however appropriate surgery might be on the basis of the histology of a mass, the anatomical limitations may dictate that surgery carries unwanted risk or is simply going to end up being too radical for a low-gravity tumour. In that circumstance, radiotherapy might be the most appropriate treatment. Please also consider simply using prednisolone for a low-grade tumour like this one appears to be.

Best wishes

Gerry

RE: Perianal subcutaneous mast cell tumour

Thanks Gerry,

We may consider referral for this case, mainly for further imaging modalities. Would you recommend using Masivet with this case?

Thanks,

Alan

RE: Perianal subcutaneous mast cell tumour

Hi

This reads like a low grade tumour. I would be trying to find a surgical solution to this problem. I don't know what the significance of the peri-renal ultrasound finding is but I would want to check that out before making a definitive plan to proceed with surgery. The case may benefit from referral to a centre with specialists in imaging, oncology and surgery.

He is radiographically normal both in the abdomen and chest cavity.

Histology:

One slide: the section shows an area of haired skin. Within the sub cutis there is a population of neoplastic

cells. Individual cells are round with a moderate amount of basophilic granular cytoplasm and a well-defined

cellular margin. The nuclei are round with a densely stippled chromatin pattern and no apparent nucleolus.

Mitotic figures are not apparent. Throughout the mass there are moderate numbers of eosinophils. The

overlying epidermis is mildly hyperplastic.

Diagnosis: Subcutaneous mast cell tumour.

Comment: The peri anal mass is a subcutaneous mast cell tumour. The Patnaik grading system for cutaneous

mast cell tumours does not apply to deeper soft tissue tumours. A recent study in Vet. Path (2011) by

Thompson et al indicates that the majority of subcutaneous mast cell tumours have a favourable prognosis,

with extended survival times and low rates of recurrence and metastasis. Full surgical excision is advised. It is

uncertain as to whether the lesion next to the kidney is related.
