My guess would be that inhaled water has caused pneumonia, which has developed into an abcess, which has burst into the pleural cavity, allowing ingress of air at the same time. This is consistent with the pyrexia, and improvement on cephacare.&lt;/p&gt;
&lt;p&gt;I would perform a pleural wash, and submit for bacteriology. Whilst awaiting the result, I&amp;#39;d put back on cephacare. If still on cephacare, I&amp;#39;d add another antibiotic, preferably one effective against anaerobes.&lt;/p&gt;
