&lt;p&gt;Thank you, Evelyn, I was hoping somebody would spot something I&amp;#39;d missed. As soon as I read your post I looked at the picture and can see what you mean - I can&amp;#39;t believe I hadn&amp;#39;t seen it myself! I will review the original DICOM picture when I&amp;#39;m back at work on Monday.&lt;/p&gt;

&lt;p&gt;Some periorbital and masticatory myopathies can be painful without initial swelling and atrophy . Retrobulbar access &amp;#39;s/fb reactions /orbital myxosarcomas etc do not always swell or cause globe deviation. Zygomatic Sialadenitis +/- mucocoele &amp;nbsp;&lt;/p&gt;
&lt;p&gt;After a month you would expect something bad to have popped out of the woodwork.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;2M antibodies ? CK&lt;/p&gt;
&lt;p&gt;Ultrasound across the orbit , and laterally above and below the zygomatic arch ?&amp;nbsp;&lt;/p&gt;
&lt;p&gt;CT if you cannot find anything another way ?&amp;nbsp;&lt;/p&gt;

&lt;p&gt;Because of its plane it is invisible on the lateral view.&lt;/p&gt;
