A was thinking about the 'holiday' bit of the post - Was someone else doing looking after the dog? was insulin not being given at all because of a less experienced keeper? and as I said before, how long had been the patient in the new dose? I think there are some more questions here than solely the glucose curve.

Sounds like an overswing to me. Did you measure the glucose every hour? If not you may have missed a lower nadir - 6-7 iu bid sounds a lot for a Pom. I would also like to get the client in to watch them administer the insulin. I never cease to be amazed by the ways then can cock this up, you can never assume they doing it right.

are you able to post the whole 12hr curve numbers and times since insulin? What happened after that initial rise?

I'd probably start with a urine culture and full bloods before changing the insulin (+ maybe an ACTH stim). I'd try and get them to feed dog food too rather than chicken.

My most difficult DM cases were ones that developed Cushings or pancreatitis at the same time..... hope this doesn't turn in to one of them!

How long had passed from the change to 6IU BID to the time of your curve?

I'd be thinking about concurrent disease eg cushings.
