This cat was first seen approx a couple of months ago with a white exudate on the medial cornea. Initially improved with debridement and topical antibiotics but doesn't seem to be improving. It's affecting lateral canthus and again just seems to be granulation tissue that fluroscein sticks to. Not really bothering cat at all. When it first presented, I assumed traumatic origin as cnjunctiva of lower eyelid looked a bit damaged too -assumed cat claw inflicted. Trouble is o unable to give topical meds. Gave convenia initially but didn't seem to alter anything and then started oral meloxicam which I think has helped a little but not a drastic improvement. If topical steroids isn't an option, would oral pred or voren/depomedrone inj be a useful alternative.
I'm no expert so take it as gospel. But you've done the correct thing taking a swab and shown as far as practical the cat currently doesn't have FHV and if in your opinion long term steroid drops are necessary to control the condition then you have your backside covered.

RE: Cat eye help please

Thank you Martin, that makes sense.
I don't think you're completely wrong but AFAIA the FHV component of the feline flu vaccines is the most effective part, albeit nothing is 100%. Compare to FCV which was 40% effective at best until the recent release of the new Purevax vaccine which is reporting 80% efficacy. But then FCV is an RNA virus so prone to mutation which FHV being a DNA virus is not.

I believe that the over-riding majority, if not all, of those grumbling FHV cats were infected from carrier mothers after their MDA expired but before they were vaccinated and themselves become carriers. If so they should show a +ve FHV PCR when swabbed when active clinical signs are present. I would think that if they managed to get to adulthood without being infected they would be pretty immune. I'm not aware of any adult vaccinated cats contracting FHV but have seen several get FCV including my own.

So if your cat was -ve for FHV PCR on a well taken occular swab I'd say you can be pretty certain it is not a carrier. Whatever we believe the epidemiology to be, if its got to adulthood with no symptoms of cat flu the chances are it won't be immunologically naive either so there is little concern of that if you're continuing steroid drops. Just check for corneal ulceration for other reasons regularly.

RE: Cat eye help please

Hi Martin,

I might be completely wrong but I thought that vaccination wasn't 100% effective against FHV-1 and although it reduced clinical signs in an outbreak it didn't prevent infection? I always thought that it explained those grumbling herpes carriers that we often see, although they could obviously have been exposed to FHV from their parents before vaccination?

RE: Cat eye help please
no doubt this cat will contract FHV eventually.
Unless that was tongue in cheek and I misunderstood, why?
thought I'd revive this thread as I have a similar case under treatment in a 1yo DSH.

It looks almost identical to the original post, same neovascularisation and oedema spreading from the lateral canthus, and my first thoughts were eosinophilic or herpetic keratitis. My colleague had seen it first and prescribed pred forte drops, after which the lesion disappeared.

It has since recurred so I've swabbed it for viruses (FHV -ve, FCV -ve and Chlamydophila -ve) and took a cytology scrape form the cornea (lots of neuterophilic inflammation but no eosinophils).

I've since started it back on pred forte drops (fluorescein negative) but am reluctant to stay on these forever as no doubt this cat will contract FHV eventually.

What are your thoughts on optimmune in these cases? Do these ever resolve or am I looking at long term treatment?
