&lt;p&gt;Of the 20 titles on the front page, 7 are clinical, the other 13 relate to other subjects. These clinical discussions can go.......well anywhere really and I start to glaze over when cascade issues are mentioned.&lt;/p&gt;
&lt;p&gt;With Julian now in place as a moderator would it be time to have a regular thread on a specific clinical subject to which we can all chip in with a clinical thought. This doesn&amp;#39;t have to be complicated, locum work has taught me that there are many different approaches and I&amp;#39;m learning all the time.&lt;/p&gt;
&lt;p&gt;My starter for 10 would be&lt;/p&gt;
&lt;p&gt;4 month old puppy with diarrhoea, what would you do?&lt;/p&gt;
&lt;p&gt;A 10 year old cat with an abdominal single mass, what to do?&lt;/p&gt;
&lt;p&gt;&amp;nbsp;I feel there is a reticence to ask simple questions for fear of sounding stupid but there is so much experience to be tapped here that it seems a shame not to use it&lt;/p&gt;

&lt;p&gt;&amp;nbsp;I think the problem in the UK is that our clinical judgement is so often restricted by this silly law. There&amp;#39;s what we want to do - and there&amp;#39;s what the cascade allows us to do - highly frustrating!&lt;/p&gt;
