I've had a cat sent to me from another vet practice to start chemotherapy for multicentric B cell lymphoma. He is 15yrs, MN and is also under treatment for septic arthritis in his left fore (diagnosed by consistent radiographs and high neutrophil

RE: Cat with multicentric b cell lymphoma

Thankyou Gerry

RE: Cat with multicentric b cell lymphoma

Reasons against: indolent disease usually non-responsive to chemo, age, concurrent morbidity

B/T typing is not of prognostic value in cats though grade of disease is: low grade (indolent) versus high grade (normal).

RE: Cat with multicentric b cell lymphoma

I have to say I read this late last night and my initial thoughts were I would give serious consideration as to whether to treat or not, but didn't know enough about the specific diagnosis to base my reply on that, so I am glad Gerry has commented.

Firstly how is the cat in itself just now and how has it been over the past year? If it well and happy, there is a significant risk the cat will feel worse with treatment and given the long term prognosis is prob not great even with chemo, is that fair on the cat?

You say the owner feels the lymph nodes have been enlarged for a year- well that doesn't fit with Dr Googles prognosis of 4-8 weeks

Does the cat have any concurrent co-morbidities eg renal disease

Why did it get septic arthritis??? Unless an obvious puncture wound, then this rings alarm bells- did the neutrophils look toxic on joint tap, I guess it could have been an inflammatory reaction rather than infection, (assume culture was done, but negative doesn't rule out infection) but even so......

What is the cat's temperament like?

Is the owner REALLY fully aware of what chemotherapy entails? What she will be putting her cat through at 15 years of age? (Doesn't mean I think it is wrong full stop, but needs to be an appropriate case)

Regarding protocols, my understanding is that there is no evidence that shows that one protocol for lymphoma is better than another, so I base my choice on costs/cat's suitability/temperament, but waiting for further typing wouldn't affect my choice (or should it Gerry Poltonnot nec talking about this case, more a general question)

Out of interest Gerry, what did you base your decision on?

RE: Cat with multicentric b cell lymphoma

It's always hard to judge without seeing a case but I have a feeling that I would not treat this case with chemotherapy (or steroids) at all.
