&lt;p&gt;Also a caveat with the fluorescein test, if seen at nostril then confirms drainage and patency, if not seen, it could be a false negative as sometimes the tear ducts drain into the oropharynx so it doesn&amp;#39;t appear at the nostril. Where this happens, I haven&amp;#39;t seen an associated dry nasal planum, Not saying its not a useful test, but be aware of pitfalls when interpreting.&lt;/p&gt;
&lt;p&gt;Is the inside of the nostril dry or is it just the planum?&lt;/p&gt;
&lt;p&gt;I&amp;#39;m just not convinced it is related to ocular pathology or even neurological. Have you done a cranial nerve assessment?&lt;/p&gt;
&lt;p&gt;Most dry crusty noses I see have a bacterial infection, fungal poss next most likely. I agree it is odd that it is so unilateral though, however the photo (to me) looks as though there is left sided involvement dorsally so maybe its not truly unilateral?&lt;/p&gt;
&lt;p&gt;Maybe &lt;a href="/members/rob-lowe" class="internal-link view-user-profile"&gt;Robert Lowe&lt;/a&gt;&amp;nbsp;could comment? Or any dermatologists out there?&lt;/p&gt;

&lt;p&gt;If it is, then the fluorescein test is diagnostic, give it a good 5 minutes, compare trickles on both nostrils.&lt;/p&gt;
&lt;p&gt;Be very careful; it stains anything, so keep it away from your silk shorts!&lt;/p&gt;
