Are you keeping it in the fridge Iain and do you discard it after a couple of days from opening? I (thought) I had a problem for a while then discovered that I was using pilocarpine instead - someone (probably me) had move the boxes around and I just reached for where I thought it was and didn't check. May be best to check you haven't done something similar.

RE: What is the best ocular local anaesthetic?

I use the proxymetacaine 0.5%, and find it sufficient to do grid keratotomies in well behaved conscious patients. Perhaps it is good for corneal local anaesthesia, but not the surrounding soft tissue? May well be higher volumes are required to be absorbed in.

A technique I use for looking for ocular FBs is lift the 3rd eyelid with a cotton bud. Seems to be fairly well tolerated in conscious patients.
