That being said, we don't have the cascade in Australia.

To me, this treatment made up by the vet will work, but it is a pretty clear breach of the cascade and I think a sledge hammer to crack a nut approach which I think is pretty irresponsible.

The cascade pretty clearly states:

The principle of the Cascade is that, if there is no suitable veterinary medicine authorised in the UK to treat a condition, the veterinary surgeon responsible for the animal may, in particular to avoid causing unacceptable suffering, treat the animal in accordance with the following sequence, in descending order of priority:

o A veterinary medicine authorised in the UK for use in another animal species or for a different condition in the same species.

o If there is no such product, the next option is either –  a medicine authorised in the UK for human use, or  a veterinary medicinal product (VMP) not authorised in the UK but authorised in another Member State (MS) for use in any animal species (in the case of a food-producing animal the medicine must be authorised in a food producing species) in accordance with an import certificate issued by the VMD.

o If there is no such product, the last option is a medicine prescribed by the veterinary surgeon responsible for treating the animal and prepared extemporaneously by a veterinary surgeon, a pharmacist or a person holding an appropriate manufacturer’s authorisation. In exceptional circumstances, medicines may be imported from Third countries through the VMD’s import scheme.

I think the prescribing vet has jumped a few of those steps.....

As for the purpose of the cascade, I thought it was to protect/help animals until drugs like cimetidine had licensed products produced at many times the cost. After that, I started to believe it was for the drug companies. I don't think the drug companies really help that perception when they not-so-subtly include statements in their advertisements "the only licensed X for use in cats". I personally see that as them trying to push the vets to stock their brand to fit within the cascade rather than trying to produce a great product for that species.

RE: Off licence ear solution - would this formula be effective?
Although it has significant flaws, surely you can see that the idea behind the cascade, to make sure vets were using licensed, tried and tested products, was a sound one? Nobody should be peddling their own home-made (cheap as chips) ear drops as a first line treatment!

RE: Off licence ear solution - would this formula be effective?
No the whole point of the cascade is to prevent vets using well proven, long in veterinary use, well tested and established human products and replace them with rebranded re-labled identical products, probably made in the same plant by the same manufacturer which end up costing sometimes ten times the price with no advantage to anyone except lots of profit for the reseller, some extra profit for the vet [increased, but same, markup] and considerable extra cost to the public [eg phenobarbitone posts passim]
Well for once I'm in complete agreement with you Anthony (except, knowing what you look like, the male model bit!) and am just as cynical about manufacturers using the cascade as a back door to increased profitability. Indeed I would challenge the RCVS/DEFRA whoever to prosecute me for using generic drugs which have been in use for generations when someone cynically decides to make a branded version of exactly the same product when the medical profession is told it must use generic versions if one is available.

RE: Off licence ear solution - would this formula be effective?
It wasn't Anthony T by any chance was it?
If I was a modvet I suppose I would say I was deeply offended, via private message, had been frightened off the forum and was going to give up vetting and become a male model, as I was disillusioned by the modern profession etc. etc. and privately burst into tears as criticism is so hurtful and unhelpful.

But I'll just respond to this:

The whole point of the cascade is to prevent people peddling their own homemade remedies when there are licensed and 'safer' products available.

I can't think of any DIY concoction I have ever used, or which has ever been used widely by anyone, but I stand to be corrected, even though I may cry and continue my new career path.

RE: Off licence ear solution - would this formula be effective?

That is a cocktail reserved for Pseudomonas.
