I just checked mine and it also says they must sign the passport at section 1 page 4 and that it is their responsibility to make sure the rabies vaccine is up to date. Sorted.

We have little post-it 'arrows' that point to the bits that need to be completed when the owner collects the passport. Always filled in before leaving the premises.

She wanted me to fill in the new owner's details on the passport but I refused and said they would have to change it later. They still didn't sign it in front of me.

Equally I had a puppy presented with a passport from Hungary with no owner details. The microchip matched the dog and the dates were OK and fitted with my observation of the dog so I let it go without reporting it although I thought twice about it. I can't see why the owner details are important so long as the passport matches the animal.

Me neither, only 'If there is a subsequent change of ownership, the client can enter the details in the next section and sign.'

Basically I spoke to the Pets Travel Scheme Helpline about a case: A breeder brought a puppy in today for Rabies vaccination and passport prior to going to Germany (already chipped). The new owner coming here to take to Germany. The Pets Travel Scheme said the easiest thing to do would be to fill in the passport with the German's owners details, then when they collect the pup to come in for a check up prior to travel to Germany and we can witness them sign the passport. That's when I said I wasn't aware we had to witness the owner sign the passport, I said I have never seen this written in any guiding documents. They said it is stated, but I couldn't see this anywhere??

I hope this helps. My read on this would be that given that practice support staff can complete section 1 there shouldn't be any requirement for you to witness this.

6. Passport sections

Sections I-III: can be completed by practice support staff or veterinarian. The owner may affix the photograph in Section II.

Section IV: is to be completed by an OV.

Doesn't say so in the ET 139 guidelines. I've never got the owner to sign, it is their responsibility. Given the client can change the name and address it is not cast in stone anyway.
