Both ear drums were found to be ruptured when he was examined under anaesthetic at the end of April. Malaseb foot soaks a couple of times/ week if dog will tolerate, and CLX wipes every day for between toes (my favourite thing for yeasty feet and nasal folds in brachys)

Apoquel would definitely be on my list of possible stuff to do, and also short courses of pred, when you've done whatever diagnostics you want to do. I wouldn't worry overly about the long term effects of pred. It's a DDB. It probably doesn't have a long term.

RE: Ear wicks in this case?

If the drums are gone it will end up with TECA s if something else does not get it first . If ears are good then a few days of ear wicks are not likely to prevent anything. If they are bad anddog is truely wicked it is a huge ask to expect owners topping up meds and/or repeat sedations to replace wicks large ear canls after he shakes them out, or even monitoring progress is going to be difficult for all concerned including patient! I'd pursue less interactive measures and caution owners may need repeat/recurrent antifungals

RE: Ear wicks in this case?

Ear wicks tend to have a habit of falling out of big ear canals.
Where the dog will not tolerate drops.

They also suggest the ear wick stays in for 5 days (2 weeks in some cases!), and where possible for the owners to top up the solution after 2-3 days to keep the wicks hydrated.

I'm just not quite sure what I'm going to be able to do for this case, as apparently he's a nightmare even to inject with dom and torb IM...

If they are intact I would go with treating allergy, but if not then you will need to intensify treatment, including properly cleaning every 1-2 weeks under video otoscopy to get them healed. If they never heal I would say it is headed for TECAs.

If you can't medicate topically then ear wicks are not going to be much use as they need the drug [antifungal in this case or antibiotic more commonly] topped up frequently.

RE: Ear wicks in this case?

I never used them, but in a TM ruptured case I wouldn't be keen to start.
