There are some suspicious small pulmonary opacities and I am waiting for results on fnab

Re: Metronomic therapy for malignant melanoma

I have a GSD patient in a similar situation but sadly the vaccine does not seem to have been helpful. I don't want to step on the toes of the specialist but having read this thread was wondering if Palladia might be an option. The owners are coming in tonight for a chat. I assume the specialist will have given the available options to the owner.

The dog is coming in because the owners feel some pain relief is needed.

Re: Metronomic therapy for malignant melanoma

There are but that doesn't mean they are any good. I would stick with 10-15mg/sqm cyclophosphamide and your NSAID of choice at standard dosages, both given once daily. The best evidence to date supports use in intermediate to high grade soft tissue sarcomas in dogs.

For future reference, are there standard drugs/doses for metronomic chemotherapy for different tumours published?

Re: Metronomic therapy for malignant melanoma

I would suggest Palladia over metronomic chemotherapy if you had the choice. There is a biological rationale for it. I have used it in a small number of cases but now have the luxury of the vaccine so don't really have a reason to reach for it any more. Unfortunately you are correct that the vaccine is only available for administration by licensed specialists. I can't tell you that metronomic therapy won't work (for that matter I can't tell you that Palladia will or that the vaccine would) so don't be afraid to try it if that is the only option you can offer.
