He presented 10 days or so again with a mass on his left shoulder (approximately prescapular area). He has a few fatty lumps, and the owner thought this was another, but was concerned it was enlarging

Re: Haemangiosarcoma

Yes we have a dog on metronomic chlorambucil and metacam that has done very well post removal of a very aggressive mammary tumour, so that would definitely be an option to consider. Thanks.

Re: Haemangiosarcoma

Would consider metronomic cyclophosphamide before giving up? Sadly the owner lost her last Labrador to a very aggressive melanoma on a toe that metastasized before she realised there was a mass. It's always the nice dogs...

Re: Haemangiosarcoma

Sounds like it would be a surgical referral case to remove something like that, but may need some advanced imaging to check extent of the mass. Before referring, I would be looking for primaries or secondaries with full abdo and cardiac ultrasound plus chest rads - no point in surgery if this is a secondary or if it's metastesised. So my approach would be to start with determining if referral for Sx an option with possible adjunctive chemo. If not, maybe the best thing for you to do would be to paliate as best you can with meloxicam or similar and otherwise let it run its course.

Re: Haemangiosarcoma

A colleague had a similar case recently, skin mass removed a few years ago and not sent for histo that had grow back... FNA as haemangiosarc. We found what we presume to be the primary in its spleen with mets to liver too. Dog was PTS.

I think you need to find out if there are any more then have a frank discussion with the owners over what you do next if anything.
