Thank you all.

RE: Glargine in cats

I wonder which bit got me 5 stars and a like for my previous post. The dissemination of my experience or the assertion that owners whose lifestyle predisposes them to type 2 diabetes, and having difficulty controlling their own condition, are less likely to have the willpower/compliance to treat their cats?!

RE: Glargine in cats

http://www.thecatclinic.com.au/info-for-vets/using-glargine-in-diabetic-cats-2
Does anyone have any good resources they can suggest regarding the use of glargine?
The information I gave in my previous post is my resource in that it came from some lecture notes I only have in hard copy. If you PM me an email address I'll try and scan and attach.
Wouldn't Prozinc be higher up the cascade?
The last cat I had on Glargine has actually now come off onto Prozinc, largely because the owners were not complying properly and wasting their money. At least Prozing has a 2 month shelf life when opened whereas Glargine has a very definite one one month life. I couldn't source the pen cartridges anymore so it was getting prohibitively expensive even though the cat is insured they still did nothing but complain.

The biggest selling point for Glargine for me is that there is a chance you can get the cat into remission if you start early enough but we weren't achieving this probably largely because of failure of compliance and probably because it had been a chronic diabetc before they got off their fat arses to bring it in.

Is it just my experience that the worst compliers are the ones who are type 2 diabetic themselves when it should be the other way round?

RE: Glargine in cats
Caninsulin appears to be to too short a duration

I am looking at switching him on to glargine
I presume that when switching, you just dose the glargine when his caninsulin dose would have been due, and then curve as you would otherwise. Does anyone have any good resources they can suggest regarding the use of glargine?

Re: Glargine in cats

I start them on 0.5iu/kg bid if fasting glucose is >20mmol/l and 0.25iu/kg bid if its less but it is recommended that starting dose does not exceed 3iu/cat bid. I love the better control it gives me and am almost prepared to ignore the cascade on this.

Downside is the expense however the tip is to buy the vials designed for the pen rather than bottles, it is much more economical in terms of wastage.

Giving it twice a day will maximise control and increase the chance for remission.

Re: Glargine in cats

I've only got one patient on Lantus. He's on half a unit bid (4.7kg cat)- seems titchy dose but seems to control him well. The owners find his demeanour and appetite better on this than Caninsulin.
