What is the Na:K ratio though? Sometimes they can be a little vague like this and electrolytes can be within normal limits

The trouble is if it's a chronic ehrlichia it can be frustratingly nonspecific. Certainly you can keep the doxy going until week 3; the Merck manual recommends imidocarb proprionate as helpful. I've used it for acute ehrlichia many times with good success in Saudi - but there may have been (and probably were) lots of other baddies in the blood in those cases; babesia, lyme, and lord knows what else. (Western Saudi Arabia doesn't have any veterinary labs that will run tests for me, so you have an advantage here that I didn't.)

I think I'd want to do a bile acid stim to look at liver function, and I might be considering an ultrasound guided Tru-Cut liver biopsy and/or collection of bile for culture, having checked clotting times first of course!
