&lt;p align="left"&gt;From the cardiologist - The most obvious finding on ultrasound was the presence of liver and gallbladder to&lt;/p&gt;
&lt;p align="left"&gt;the right of the heart, presumably within the pericardium. This, along with the dogs&lt;/p&gt;
&lt;p align="left"&gt;deformed rib-cage, did not make for a normal echocardiographic examination.&lt;/p&gt;
&lt;p align="left"&gt;The left atrium is not significantly enlarged - Ao 24, LA 31mm. The LV function&lt;/p&gt;
&lt;p align="left"&gt;looks reasonable, but the end systolic dimension is increased as would be normal with&lt;/p&gt;
&lt;p align="left"&gt;a PDA. PDA flow can be seen with pressure gradient of 108/70 ( systolic/diastolic )&lt;/p&gt;
&lt;p align="left"&gt;shunting from left to right. I could not assess the ductal morphology at all. I cannot&lt;/p&gt;
&lt;p align="left"&gt;detect any pulmonic stenosis, but it was very poor image. I also cannot definitely rule&lt;/p&gt;
&lt;p align="left"&gt;out other defects on the right side due the difficulty in obtaining images. Looking&lt;/p&gt;
&lt;p align="left"&gt;back at my 2-D images, I would like to rule out the presence of a double-chambered&lt;/p&gt;
&lt;p align="left"&gt;RV, as there appears to be some narrowing in the mid RV region ( if the dog is&lt;/p&gt;
&lt;p&gt;referred for surgery I will mention this as it would be assessed again ).&lt;/p&gt;

&lt;p&gt;The general public seem to think a vet is a vet. I bet they would complain pretty loudly if they were seen in hospital by a &amp;#39;teenage&amp;#39; doctor with poor English skills &lt;img src="https://www.vetsurgeon.org/emoticons/v2/tongue-in-cheek.gif" alt="Tongue-in-cheek" /&gt;.&lt;/p&gt;
&lt;p&gt;Of course there are lots of teenage looking doctors with poor English skills.&lt;/p&gt;

&lt;p&gt;I&amp;#39;d agree that a pansystolic murmur audible across the whole chest is unlikely to have appeared since the vaccination course. Did the owner notice the abnormal chest shape or was it pointed out to her by the breeder? Is there any chance the owner may have been advised of the problems but didn&amp;#39;t realise (or chose to ignore) due to language differences?&lt;/p&gt;
&lt;p&gt;This evening I was perusing Trade-It whilst waiting in the chippy. There are are a vast number of puppies for sale described as &amp;#39;vet checked&amp;#39;. &lt;p&gt;I find it highly unlikely a 4/6 pansystolic murmur can just appear out of nowhere in the 3 weeks since the rabies vaccine, unless the heart wasn&amp;#39;t checked?
