so an update, I went back into the joint and found the patella groove was horrid but also there was a cranial cruciate ligament feathering at one edge , perhaps 5% of the tendon gone - debrided this tiny bit about 1 mm and place a extracapsular suture; at the time thought this was overkill as there was no joint /lig laxity.

B***ger me the do is walking normally 10 days post surgery.

Patellar desmitis? Is the patellar ligament thickened on the med-lat radiograph? Lumbosacral IVDD?

no tension band , joint taps at pin removal were clear, no spinal pain or pain anywhere other than stifle but he does act totally differently when with me compared to being at home, but certainly after a lot of stifle manipulation he did not want to stand on leg,

Couple of random/quick thoughts

1. was he re-xrayed at pin removal? Was there a tension band wire that may be irritating the tissues (not sure if removed with the pin)

2. joint taps in case some rumbling low grade infection

3. some sort of spinal problem. Are there any neurological deficits or spinal pain

dog is terrified of water have suggested but owner refuses, does do passive exercises with him though.

Did the dog have a course of hydrotherapy after the original op? May be something to trial to try and increase quadriceps strength.

I re-examined the dog last night , at home he shuffles on his bottom now not using either hind leg unless stimulated to play with other dog when he does get up but stifle flicks laterally as he walks- but this is hard to see on the video she brought me, there is some luxation of right hind too so I think this is why he isn't walking on 3 legs now. on exam the only place he reacts to is when I manipulate the stifle in each leg left more than right , left leg ( the problem one) does not have cranial drawer or an meniscal pop but of course we are dealing with tiny leg, patella is about 2.5mm wide by 5 mm long. there is no luxation palpable but there is a slight jumping sensation as though the patella is not running smoothly. I have booked it back in for exploration of the joint , I wonder if it has developed an overgrowth of fibrocartilage where the groove was deepened and this is stopping normal function.

Does the lameness exam localise to the stifle? Could there be another source of pain at a different location, or if definitely stifle could there be meniscal issue? Joint effusion?
