&lt;p&gt;&amp;nbsp;&lt;/p&gt;
&lt;p&gt;In the end it was vetericyn that made the difference. &amp;nbsp;Mamuka and silver gels did not seem to help, but were in the face of the infection. &amp;nbsp;Seemed to make it too wet and over granulate.&lt;/p&gt;

I&amp;#39;ve blunt-dissected a triangular area on the lateral surface, freshened the edges, and mattress sutured down to the axilla.If this does well, I&amp;#39;ll repeat on the medial surface in another 6 weeks. In the mean time, there&amp;#39;s healthy granulation tissue, and I&amp;#39;ll keep on manuka, antibiosis and metacam.&lt;/p&gt;

&lt;p&gt;I think most of the suggestions seem reasonable. Cats are slower to heal than dogs so although leaving to 2nd intention (esp when funds low etc) is reasonable, it will take quite some time. Flushing/debriding the wound to get it nice and clean would be the first step whatever you decide. Re skin stretching - its hard to get this to work properly, don&amp;#39;t order in Pav&amp;#39;s kit as you can get the same equipment from Hobbycraft for a few pounds. Use adhesive velcro pads if you want to try this (in fact message me if possible and I&amp;#39;ll talk you through what to buy/how to do it). If you want to do something fancy then I would do bilateral inguinal skin fold advancement. The abstract for the dog technique is:&lt;/p&gt;

&lt;p&gt;Wrt primary or secondary (leaving the gran bed intact) closure - I&amp;#39;d leave the gran bed if healthy and close over it, it will be q exudative so you will need to put a drain in. I would not graft and I&amp;#39;m not a fan of stent sutures but they have their place. If you undermine lots you may even be able to close this simply as it&amp;#39;s not any bigger than the average injection site sarcoma resection which is usually closed as a simple midline scar.&lt;/p&gt;
&lt;p&gt;Re the axillary wound - thoracodorsal APF&amp;#39;s are v good for these but they can be unforgiving. If you don&amp;#39;t preserve the nutrient artery (thoracodorsal) then the flap will fail and this can be catastrophic so take your time, use drains. Don&amp;#39;t bandage it, don&amp;#39;t try to &amp;quot;tack it down&amp;quot; and ensure you maintain BP with IVFT at surgical rates. Omentalisation works but I&amp;#39;ve not had a problem in cases that I haven&amp;#39;t omentalised.&lt;/p&gt;

The suture will loosen overnight as the skin stretches, and you can loosen the lead ball, move it along the suture material and reapply tension each day. You can achieve quite amazing reduction in defects with this method, and it&amp;#39;s much less of a faff than velcro straps.&amp;nbsp;&lt;/p&gt;

&lt;p&gt;As an aside, I know axillary wounds can be a nightmare to close, but Pavletic claims to almost never need to omentalise these. His approach is always to deal with the tension, either by closing in multiple layers or with walking sutures etc, or using a flap.&lt;/p&gt;

&lt;p&gt;If there&amp;#39;s definitely no more healing to be had (can always leave alone and come back to in 6 weeks if not sure), then I&amp;#39;d personally do a thoracodorsal axial pattern flap to give a tension free closure. Quite easy to do in a cat with instructions open in front of you and a little forward planning even if your first time - just make sure skin is in a &amp;quot;neutral&amp;quot; position before starting to cut (pull it up and let it drop down again and make sure not caught under the recumbent surgical patient) and use a sterile surgical pen to draw the vessel and where your cuts will be and measure the legth required using a piece of something non-stretchy like string... and clip double what you reckon you&amp;#39;ll need &lt;img src="https://www.vetsurgeon.org/emoticons/v2/Winking_smiley.gif" alt="Wink" /&gt;&lt;/p&gt;
&lt;p&gt;I&amp;#39;ve always been concerend at causing peritonitis by stealing some omentum and doing that techniuqe, but I suspect it&amp;#39;s a pretty low risk and probably pretty straightforward also.&lt;/p&gt;
