I don&amp;#39;t think the age is at all relevant except with a Great Dane you need to keep adjusting doses as the dog grows. As others have suggested start low and increase the dose every 5 days or so. I normally target 400-600ug/kg SID. I would certainly expect positive scrapes on ivomec/milbemax for several months after starting treatment. &lt;p&gt;To my knowledge, the allele you can test for (i.e. the common one to cause problems) has never been reported in a Great Dane and I think it would be safe to assume that in a pedigree Great Dane this specific genetic abnormality is highly unlikely to be present, as such I wouldn&amp;#39;t consider gene testing.&lt;/p&gt;

&lt;p&gt;I agree here. If it is severe then I would not personally hesistate to start on oral ivermectin at a dose of 0.1mg/kg q24hrs and titrate up over a week to 0.5mg/kg q24hrs assuming no clinical signs of toxicity present.
