I have a 14y old MN diabetic Siamese as a patient. He's been in remission for a few months although his latest fructosamine had risen to 403 (starved blood glucose was 8.7mmol/l)

Re: Diabetic cat with IBD-like signs

Totally agree with pancreatitis as an underlying issue here

Re: Diabetic cat with IBD-like signs
Actually all my cats love me, they want a dominant alpha-male.
We found him dead, soaking wet, stuck in a bush the next morning. I assumed he'd died of hypothermia brought on by a hypoglycaemic episode as he would try and push himself into a corner when he had one (I confess I wasn't as good at managing diabetic patients back then as I am now). However I guess he could have committed suicide!

Actually all my cats love me, they want a dominant alpha-male.

Re: Diabetic cat with IBD-like signs
as he also had urolithiasis.
Given the age of onset it is likely that it is in this case part of a chronic triaditis with acute on chronic pancreatitis. I would attempt to manage the DM best you can and control the IBD/pancreatitis with diet and other supportive therapy along with the CKD but give a guarded prognosis.

Been there with one of my own cats it took a lot of juggling diets especially as he also had urolithiasis.

Re: Diabetic cat with IBD-like signs

Thanks Kate. Just as an update this cat has indeed slipped back into a diabetic state, post-prandial glucose having climbed to 21mmol/l.

He's back on caninsulin and hopefully going for an ultrasound next week to rule out other GI etc pathology.

Re: Diabetic cat with IBD-like signs

Looks to me like he has pancreatitis, may be a chronic one with an acute flare up given the history of DM. Tx with pain analgesics/anti-emetics, diet wise I wouldn't change at this stage as this could tip him back into being diabtic again, so monitor closely for hyperglycaemia developing- home collected urine samples are the best way as rules out any stress from the clinic or sampling. Also monitor for development of EPI in the future.

Re: Diabetic cat with IBD-like signs

Heart sounds ok - folate/cobalamin were normal so I would have thought that ruled out SIBO. I haven't tried an exclusion diet because it's been on diabetic food and its fructosamine has started creeping up anyway so I was worried about pushing it into a full out diabetic state (though of course IBD can cauyse insulin resistance anyway - can't win!)

Re: Diabetic cat with IBD-like signs

Not a clin path expert by any means, but glob means inflammation, and high urea with normal creatinine means pre renal. What does it's heart sound like? Could the occasional soft faeces mean bacterial overgrowth 2y to too much glucose being secreted into intestines/known predisposition of diabetics to infections. Have you tried an exclusion diet?
