Sorry didn't read the OP properly, I guess my suggestion is not overly helpful other than the amount you need is so tiny they may have more chance of poking the little blighter in the eye with a cotton but than trying to hold it open while they drop in the ointment. However Protopic is expensive and in limited supply.

I had one such case and an ophthalmologist suggest Tacrolimus (Protopic). The owner put it in once a day with a dab on a cotton bud. It transformed the eye which was non-responsive to cyclosporin but read the comments in Andy's link above.

but in brief, yes oral cyclosporine seems to have equivalent action to topical in most dogs so would be worth a try.
