Does anyone have any suggestions for a cat I saw with very scabby lesions on both his pinnas. The scabs can be removed to reveal raw skin. He is pruritic with it and the concave surface of each pinna is erythematous. No mites on microscopy

Re: cat with scabby lesions on pinnas

I take it all back and am eating humble pie as I type. Maybe it's getting more common. my new one seems to have been triggered by convenia given last November, and bizarrely both cefalexin and clav amoxy made the signs worse... so poss drug induced.

The picture you posted looked almost classical for PF to me...and i have never seen a case of dermatophytosis that looked like that - but again i almost never diagnose dermatophytosis .

Once had a run of 4 osteosarcomas in 2 weeks - since then just one or two a year.... there's no accounting for chance, I guess!

Re: cat with scabby lesions on pinnas

The biopsies gave the definitive diagnosis of [drum roll...] pemphigus foliaceus.

Re: cat with scabby lesions on pinnas

Could this be secondary infection to rabbit ear mites? Our old tom cat used to get these when he raided warrens. Big black crawly things, they would only be there for a few hours but drove him mad. used to paint fipronil onto his ears weekly in bunny season, did the trick.

Re: cat with scabby lesions on pinnas
Apologies Chris, I am just probably a bit over sensitive regarding rare causes of things as have had a run of colleagues seeming to test for the weird and wonderful before giving simple common treatments a chance to actually work!

I have never seen a case of pemphigus in 18 years- has anyone?
Several cases, actually, over about the same time period.

BUT I've only ever diagnosed 2 de novo cases of Cushings all by myself, in spite of looking hard, and one of those was my own dog!

I didn't think pemphigus was particularly rare in cats but clearly everyone else does....

Go figure....!

Yes - about a month ago! (so it sprang to mind) - and it looked exactly like the ear in the photo. I have got a picture of it at work I'll try to find and upload. Like Chris I was just suggesting it as a differential, not a definitive diagnosis.

Re: cat with scabby lesions on pinnas

no probs, just probably being a bit sensetive myself today.

I have had 1 cat with pemphigus. as well as my own dog with severe PF for 5 years pretty nasty. I seem to remember in cats it affected the ear pinnae but also the toes causing interdig crusting, confirmed on biopsy. Totally agree with you regarding treat the simple first.

Re: cat with scabby lesions on pinnas

Apologies Chris, I am just probably a bit over sensitive regarding rare causes of things as have had a run of colleagues seeming to test for the weird and wonderful before giving simple common treatments a chance to actually work!

+1 for pemphigus, a biopsy would help but isnt always diagnostic.

I am suggesting that pemphigus is on the list as per other posts, I am very aware that common things occur commonly but simple tests have been done and awaiting results. just adding a differential to the list not suggesting that this is def pemphigus. of course we havent been told the age of the cat, drug history etc. Just suggesting if simple things arent coming to a conclusion or improving then biopsy would be worth looking at to check for various causes.

Re: cat with scabby lesions on pinnas

Looks very much like dermatophytosis to me, we see a lot of it here in Hong Kong and the pinnae are a favourite place.
