I suspect consent would be required if it were to be used in anything other than in shoebox animals!

I have a standard form which I passed by the VDS and RCVS and covers just about every eventually. It is rarely used in practice and only when I'm using something that is virtually contraindicated but is the only viable treatment. If I remember I get owners of exotics to sign a form when they first arrive which says that from time to time I may wish to use medication on their pets which is not licenced in that species (i.e.nearly everything!) and this is ongoing so they don't have to sign every time. Otherwise it is verbal consent which is noted in the clinical records.

We have a standard form that comes with Ezvetpro and use that just enough to (hopefully) keep the VMD inspector happy. I do explain to clients when I go off licence but for Pardale plus NSAI no form filling!

Generally I would hope there is enough trust to make much of the form filling unnecessary but there's nowt as queer as folks when it comes to litigation!

I've recently become aware of our standard forms being inadequate when using off-licence combinations of licensed medications. A common one is Pardale with a NSAID but I guess it could extend to other scenarios. For example steroids within 24h of an NSAID (eg pyrexic dog given meloxicam one day then it transpires it has IMHA the next day) or Advocate given to a dog recently wormed with Milbemax even though it is common practice.

Does anyone have a form for this stuff? Or care????

We have a fairly standard off licence form - owner details, animal details, product details etc. Wording then along the lines of "It has been explained to me that x is not a veterinary licenced product and I have been made aware of the possible side effects." (We give owners a BSAVA hand out about the particular drug involved to cover that). There is also something on there about the drug being chosen for this patient as there is not a suitable licenced alternative available, and then the owner asked to sign. Fairly simple, though sorry for not having the exact wording. We use these routinely for every off licence drug use and our computer system flags these up as we invoice/label them so we know to get consent.
