 If anyone has and is willing to share their off-label disclaimer they get clients to sign when they&amp;#39;re ivermectin (or anything else off label), we&amp;#39;re currently in the middle of drafting such a document. It is rarely used in practice and only when I&amp;#39;m using something that is virtually contraindicated but is the only viable treatment. If I remember I get owners of exotics to sign a form when they first arrive which says that from time to time I may wish to use medication on their pets which is not licenced in that species (i.e.nearly everything!) and this is ongoing so they don&amp;#39;t have to sign every time. I do explain to clients when I go off licence but for Pardale plus NSAI no form filling! &lt;/p&gt;

A common one is Pardale with a NSAID but I guess it could extend to other scenarios. For example steroids within 24h of an NSAID (eg pyrexic dog given meloxicam one day then it transpires it has IMHA the next day) or Advocate given to a dog recently wormed with Milbemax even though it is common practice.&amp;nbsp;&lt;/p&gt;

There is also something on there about the drug being chosen for this patient as there is not a suitable licenced alternative available, and then the owner asked to sign. Fairly simple, though sorry for not having the exact wording.
