Just wondered about how frequently people use Aludex and how worried people are about effects from continued use? The reason I ask is I recently took on a case which is new to me but has been treated quite a bit already for skin problems

Re: Aludex
Is hideously expensive however particularly in bigger dog... is there really a clear enough advantage over cheap oral ivermectin drench to justify the expense?
I don't know to be honest. You need to educate them properly, and if they can't or won't do it, then do it in-clinic.

the second most likely reason for recrudescence is the use of corticosteroids, even topically, in an animal that has already had demodecosis. I anm pragmatic, and appreciate that you can be caught between a rock and a hard place, especially with an itchy dog, and would simply say you need to have the demodex discussion with the client before you use any.

I would feel that ANY demodex found on a skin scrape of a dog with appropriate clinical signs should be treated, this seems to be the current dermatologist position based on my last CPD course in this area. If you consider the tiny volume of skin sampled in the average scrape, this makes sense.

A final point, for monitoring purposes, do hair plucks. Demodex are easy to see on these, and if you see them, there is no need toscrape. If you don't see them, you should confirm with a scrape, this can save a fair bit of time at the beginning.

Re: Aludex
I don't know about how often you should use Aludex but I would be questioning just how bad this dermatitis is, given it's a bull breed some demodex is almost always present but transient and what do you mean by 'lots of' demodex? - 2-3 per LPF may not be significant in this breed. Then if demodecosis is the real problem and If it really is that bad I'd be asking if they are using the Aludex effectively and /or what is depressing the immune system of a young dog so much to make it apparently resistant to Aludex - has someone also been using steroids?
A very short course of steroids was prescribed 6 months ago and then the dog has had some fuciderm since. The other vet who had seen it was starting to wonder if there was another reason for immunosuppression though.
Are they using topicals regularly? Monthly advocate has shown some efficacy with preventing flare ups from demodecosis cases if used every 4 weeks. Was an article in JSAP about 1 1/2 years ago I think.
Some advocate has been used but not in conjunction with the aludex. This is probably a silly question but I didn't think I could use advocate actually at the same time as Aludex because it says under contraindications for Aludex ' Do not use simultaneously with other ectoparsiticide preparations'.
The PDSA use Promeris Duo for juvenile demodex (monthly) They see a lot of cases especially in Staffies (could be a skewed statistic though !!)

The problem they have is after 2 treatments the dogs look so much better that they don't re-present and they flare up again.

Anecdotally the skin nearly always looks worse for a few days after Promeris application, so don't be put off by this, especially if the client phones up the next day complaining the drug hasn't worked

    Neil
