Why would it be any more likely to worsen with surgery than with medical treatment?

From my own experience - my own cat - unilateral thyroidectomy at about 13yrs, contralateral gland affected at 17 years old, couldn't tolerate meds, mildly increased renal parameters (when you could actually find a nurse willing to help take a blood sample from a hissing spitting nightmare of a cat), BP - you must be joking, non-compliant owner , surgery the only real option - renal parameters actually went back to normal post op and remained so until she died at 20!

Re: To op or not to op?

It was a really difficult cat to take bloods off from at 1st, it's 1st ever bloods it needed sedation for! So bloods havent been done as often as wold be able to normally. Most recently did go hypothyroid on meds, so gradually reduced down to 2.5 mg felimazole bid, due bloods again to see how doing on this dose now (had increased in past due to clinical assessment). Mild increase in liver enzymes. Mild inc in urea which has worsened since diagnosed, but literally still only just above normal (havent got values to hand as not at work :-) ). SG intermediate as above, no proteinurea on dipstick. BP approx 160 mmHg but it did get a bit upset with this.

Thinking repeat bloods and see how they are now and then plan for sx as such a young cat really. Have warned owner about poss of reccurance of problem if other galnd becomes diseased or due to ectopic thyroid tissue. Have also warned about poss hypoparathyroidism possibilty (but less likely if unilateral gland op?)

Any concurrent conditions? I would probably operate, and especially so if the cat is hard to tablet/ owners not compliant.

Do the owners know about risk of rogue thyroid tissue so even removing obvious goitre may not cure?? and risk of parathyroid distubances post op??

Sorry for all the questions but it's late on a Friday night and my brain is fried.
