High is diagnostic but even an insulin value in the normal range&amp;nbsp;is considered abnormal as it is inappropriate for a hypoglycaemic state. On preds the values may not be reliable though. I would be highly suspicious of an insulinoma from repeated low measurements (3.4mmol/l is usually used as the cut off value).&lt;/p&gt;
&lt;p&gt;I prefer to use diazoxide for control where possible - more expensive than preds but it is a more specific therapy. Surgery is an option but many insulinomas are microadenomas so accurate localisation for removal can be impossible.&lt;/p&gt;
